This talk will provide an overview of the normative and empirical literature on demand-side cost sharing in health insurance. After a brief review of the basic tradeoff between efficiency and risk protection, it will address considerations of multiple goods, consumer errors in valuing health care and “offset effects.” The practice of selective contracting on the supply side is an alternative to demand-side cost sharing. The talk will review the argument for selective contracting and relate this supply-side policy to the question of optimal demand-side cost sharing.
